Health Fair Pre-Admit Forms & Curbside Blood Draw Map

Unfortunately, due to the lack of resources, i.e. staffing and supplies, and the complex integration of our new EMR system, Minidoka Memorial Hospital will not be holding their Annual Health Fair blood draws this year.

To help patients and community members continue to keep up with their annual blood work we continue to offer our Direct Access Patient lab testing which allows you to have many of the same blood tests of your choice at the similar reduced price to our Health Fair tests. Like the Health Fair blood tests no physician orders are needed, and the results are mailed directly to you upon completion for you to share with your health care provider.

Direct Access Lab tests can be done Monday-Friday at your convenience.  Keep in mind, that for certain tests you will need to fast for 8 to 12 hours prior to coming in for blood work. For more information and a copy of the Direct Access Patient lab tests offered visit lab services at  www.minidokamemorial.org
