TAKE OUR

community health survey

Minidoka Memorial Hospital is conducting a community health needs assessment (CHNA) to identify local health needs and to further our mission of providing safe, friendly, professional, and high quality care.

The 2020 CHNA report informs Minidoka Memorial leadership, public health partners, and community stakeholders of the significant health needs in our community. This allows our hospital and local partners to develop strategies that leverage Minidoka Memorial and community resources to address those needs throughout the Mini-Cassia area.

Please help your community by completing this quick 5 minute community health survey.
